On the premise of complying with national laws, regulations and the "Municipal Housekeeping Service Industry Convention", in order to protect their respective legitimate rights and interests from infringement, all parties voluntarily sign and consciously abide by this contract:

1. Service content

Please select the required service content from the following items, or fill in other new service items in brackets ().

1. Housework; 2. Babysitting; 3. Taking care of children; 4. Taking care of the elderly (male, female); 5. Taking care of the elderly (unable to take care of themselves: male, female); 6. Taking care of patients (male) , female); 7. Take care of patients (semi-autonomous: male, female); 8. Take care of patients (unable to take care of themselves: male, female); 9. Stay with the hospital; 10. Pick up and drop off students.

2. Rights and responsibilities of Party C

1. Provide domestic attendants to Party A. Establish a home attendant transportation base and maintain regular contact. The source of the home attendant is clear, the identity is clear, and the procedures are complete. Party A will be charged a one-time home attendant usage fee and handling fee of ***200 yuan.

2. Party C collects a deposit of 300 yuan from Party A. After the service period of the home attendant expires, Party A will safely return the home attendant to Party C, and after confirming that the wages of the home attendant during the service period have been settled and there are no other labor disputes, Party C will return the deposit to Party A in full.

3. The clothes and prepaid wages voluntarily donated by Party A to the domestic helper are voluntary acts of both parties, and Party C is not responsible for recovery. A monthly housekeeping service management fee of _____ yuan is charged.

4. For newly arrived home attendants and those who return to the company after their term has expired, the company will provide professional ethics education and professional retraining as appropriate, and re-arrange their employment.

3. Rights and Responsibilities of Party A

1. Pay the domestic service staff a full monthly salary of _______ yuan in a timely manner (the specific salary amount will be determined after signing this contract) time, determined by negotiation between both parties). The rest days for domestic workers shall be no less than three days per month. If they are unable to rest during national festivals and holidays, the two parties will negotiate an appropriate salary increase. Domestic workers may not be owed or withheld wages.

2. After paying the home attendant usage fee and handling fee, you can enjoy the opportunity to change the home attendant three times for free within one year.

3. Provide free normal room and board to domestic servants. They are not allowed to live in the same room with adults of the opposite sex. And we must treat others equally, do not discriminate or mistreat others, and ensure the safety of personal and property during the service of domestic attendants.

Obligated to provide guidance to domestic servants in their domestic service work.

4. When a domestic attendant is suddenly ill or otherwise injured, necessary measures should be taken to treat the illness and save the patient. If an injury occurs due to the work stipulated in the contract, the domestic attendant shall bear the appropriate medical and medical expenses.

5. Household attendants shall not be forced to engage in work other than those under the contract, shall not be taken to other provinces or cities, and shall not transfer domestic attendants to others without permission.

6. According to the relevant regulations of the city, apply for "Temporary Residence Permit", "Marriage and Childbirth Certificate", etc. for domestic servants.

7. The right to request and arrange for the home attendant to re-take a physical examination. If the physical examination fails, the cost will be borne by the home attendant. Otherwise, Party A will bear the expense.

8. The person has the right to refuse the home attendant to bring fellow villagers, relatives and friends into the home. The right to refuse the use of home long distance telephone calls by the home attendant.

9. Party A should keep its valuables such as handcrafts, jewelry, cash and so on at home properly. For financial losses caused by domestic servants due to work errors, they should negotiate with them or seek legal help. Do not infringe on the personal rights of home attendants.

10. If a domestic attendant is found to be suffering from a disease that makes her unable to perform housekeeping services, she should be sent back to Party C promptly and the contract should be terminated.

IV. Rights and Responsibilities of Party B

1. Respect and safeguard the legal rights of Party A and Party C, consciously perform the housekeeping service items stipulated in this contract, and provide dedicated services.

2. Have the right to receive labor wages on a monthly and on time basis. There should be no less than three rest days per month. If you need to work overtime, you have the right to negotiate with Party A to increase wages.

3. The right to refuse all unreasonable requests made by Party A. Have the right to protect their personal and property rights from infringement.
